Lakeville High School is seeking candidates to be honored during halftime at the homecoming football game each year as "Distinguished Alumni." You may know someone who is deserving of this award. Please complete the "Distinguished Alumni Nomination Form" to nominate a deserving candidate. Nominations will be judged on the following criteria:
• The candidate must have contributed to society in an exemplary manner.
• The candidate must have achieved outstanding success in the following: business, a profession, the arts, humanitarian efforts, or community service.
• The candidate must be a good role model for students.
• The candidate must be a graduate of Lakeville North or Lakeville South High School.
